Terms and conditions:
- Discount is valid for full priced programs and 3+1 packages only.
- Advanced booking is required. Exilis Ultra 360 and EmSculpt Neo treatments are strictly by appointment only.
- Other packages, prepaid programs, and third-party vouchers are excluded.
- To avail, a Metrobank cardholder must charge the total bill to their qualified Metrobank credit card through Jan Medical Group's Point-of-Sale Terminal.
- Only one (1) qualified Metrobank credit card will be accepted per cardholder, per day, even if he or she has multiple qualified Metrobank credit cards.
- Splitting of transactions is not allowed. Splitting of transactions is defined as an intentional act of dividing a merchant transaction into multiple charge slips to redeem more than one reward incentive. A presumption of splitting of transactions arises when there are two (2) or more transactions in one merchant outlet or establishment over one and the same product, and the interval of these transactions were made within 5-10 minutes.
How to Avail:

a. Visit website: www.janmedicalgroup.ph.

b. Existing users can log-into their account. New users must sign-up.

c. In the menu bar:
- For Shape Ultra and Shape Sustain programs: Click "Weight Management", then "Shape Programs". Click "Discover" under Shape Ultra or Shape Sustain", then click “Begin Weight Loss”.
- For Exilis Ultra 360 and EmSculpt Neo treatments: Click "Women's Health", then choose "Exilis Ultra 360" or "EmSculpt Neo"
- Then click "Book Now". Choose date and time.
- Select your preferred treatment and click "Book Now".
- Select your preferred and time then choose 3+1 sessions and click "Next"
- Complete the booking form and click "Add to Cart" or "Book now".
d. Under “Promo Code” type MHEREONLINE and click “Apply.”

e. Type In your contact details. Select your preferred delivery method.

f. Under “Payment, click “Place Order and Pay”.

g. Enter your Metrobank credit card details to settle the payment.
The promo can be availed at 2/F Unit E Bengar Bldg., 447 Del Monte Ave., cor. Biak Na Bato St. Manresa, Quezon City.
The promo is non-transferable, non-cumulative, cannot be exchanged for cash or other products.
The promo is not valid in conjunction with other promos of Metrobank and Jan Medical Group.
In the purchase of goods and services in connection with this promo, senior citizens or persons with disability (PWD) can either avail of the promo or the discount provided under the Expanded Senior Citizens Act 2010 or Magna Carta for Persons with Disability, whichever is higher, if applicable.
The use of the Metrobank Credit Card in connection with this promo is subject to the Terms and Conditions governing the issuance and use of Metrobank Credit Cards. In such a dispute, Jan Medical Group shall have no liability.
Metrobank reserves the right to disqualify a previously approved transaction subsequently cancelled or charged back within the promo period. Should a cardholder receive the discount, but was later on deemed unqualified for not meeting the requirements of the promo due to disputes arising from, but not limited to, erroneous, invalid, fraudulent or unauthorized transactions, the cost of the discount shall be charged by Metrobank to the cardholder’s credit card account.
Any amendment in the terms and conditions is subject to DTI approval.
The terms and conditions governing the issuance and use of Metrobank Credit Cards, reminders and other provisions contained in the card carrier, statement of account, charge slips and other documents or instruments, which are made an integral part hereof by reference, shall likewise be resorted to in instances where they are applicable in this promo.
Any dispute concerning the products or services, including pricing, related to the promo shall be settled directly between the cardholder and Jan Medical Group, with the concurrence of the DTI. Metrobank shall be free from any liability in the event of such dispute.
In case of dispute in respect of Metrobank only and the cardholder’s availment of the promo, the decision of Metrobank, with the concurrence of the DTI, shall be considered final.
By joining or availing of this promo, the cardholder confirms that he/she has read, understood and agreed to the promo mechanics and its terms & conditions.
